Position Summary –
The Life Coach (LC) will work directly with the participants in Transitional Housing Program-Plus (THP-Plus) for former foster youth, ages 18-24 years old. The THP-Plus LC will work closely with the youth, team members, and County partners to develop and follow through on strength-based, individualized plans for helping youth meet needs across a wide variety of life areas. The THP-Plus LC will provide case management services in the form of on-going support, mentoring, assisting participants with goal setting and monitoring of participant’s progress toward goals.
