Basic Qualifications:Experience• Minimum four (4) years of environmental health and safety administration / management or work place safety, disaster management experience in a health care setting. Position Summary –
The Life Coach (LC) will work directly with the participants in Transitional Housing Program-Plus (THP-Plus) for former foster youth, ages 18-24 years old. The THP-Plus LC will work closely with the youth, team members, and County partners to develop and follow through on strength-based, individualized plans for helping youth meet needs across a wide variety of life areas. The THP-Plus LC will provide case management services in the form of on-going support, mentoring, assisting participants with goal setting and monitoring of participant’s progress toward goals. The THP-Plus LC will assist the participants in a strength based approach to implement independent living skills, (i.e., money management, job seeking, volunteering in the community) and obtaining their educational goals. The THP-Plus LC will work with the individual youth to mentor and teach these life skills in various settings including at the youth’s apartment, the office, and in the community.
